1. Support individuals as needed in daily living activities such as personal and health management, communication, self-advocacy, realizing personal and educational goals, meal planning and preparation, shopping, housekeeping, laundry, basic home maintenance and safety, recreation, home management, financial management and decision making.
2. Promote inclusion for individuals through typical community activities.
3. Support individuals as needed in developing friendships and establishing desired relationships with peers.
4. Participate in the development of Individual Habilitation Plans and in support circle meetings as needed.
5. Complete household chores to the extent necessary to maintain reasonable standards of health, sanitation, safety and building maintenance at all times.
6. Assist individuals in receiving needed medical, dental and counseling services.
7. Communicate with individuals' families and day and employment program personnel as assigned by supervisor.
8. Assist in maintaining records of medical/dental treatment, petty cash funds and individual funds entrusted to the company as assigned by supervisor.
9. Serve as shift supervisor as assigned by supervisor.
10. Administer medication to individuals who need support taking medication.
11. Attend team meetings and training programs.
12. Transport individuals in personal and company vehicles as required.
13. Cooperate with company and regulatory agency personnel during any inspection or investigation.
14. Prepare required and requested reports.
15. Perform other such duties as may be assigned for providing excellent customer service and for the most effective operation of the company.
