Peritoneal Dialysis Nurse- Area Coordinator

Duties and Responsibilities:

The duties of the Home Dialysis Nurse encompass the patient training as follows,

 
Candidate Selection:

 
Evaluation of Peritoneal Dialysis candidates.
Ability to evaluate home environment in order to meet treatment specifications.
Works closely with nephrologists and social worker to screen patients for program. 
 

Patient Care:


-  Develops and maintain nursing care plans.
 

-  Prepare patients to assure primary responsibility for own health maintenance.
Orient patients to the peritoneal dialysis program.
 

-  Train the patient in peritoneal dialysis techniques.
 

-  Provide written resources pertaining to peritoneal dialysis.
 

-  Monitor, order and mail supplies as needed for the home dialysis patient.
 

-  Coordinate care while the home dialysis patient is hospitalized.
 

-  Perform tubing changes for CAPD/CCPD patients.
 

-  Provide emergency care as needed.
 

-  Make periodic visits to the home for surveillance of performance and adaptation.
 

-  Maintain records. Evaluate patient monthly in clinic i.e., blood pressure, weight, adequacy, and ongoing patient education. 


-  Quality Assurance / Financial Management


-  Charges for patient care services according to RCP policy.
 

-  Complete flow sheets and appropriate charge documentation.
 

-  Order initial hoe dialysis supplies. Monitor usage.
 

-  Participate in quality assurance program and cost containment analysis.


-  Communication and Patient Service Skills



-  Ability to articulate instruction for patient health improvement, problem solving or a  mechanical problem when speaking with patients over the phone.
 

-  Thorough knowledge of ESRD, CAPD/CCPD and hemodialysis for patient training.
 

-  Must provide detailed and accurate documentation of all patient communications, and health status changes in medical file.
 

-  Maintain contact with patient physician or nurse practitioner.
 

-  Follow orders, protocol and procedures as assigned by either physician or the immediate supervisor.
 

-  Administer patient quality care in a considerate, respectful manner.


-  Other Duties as Assigned


-  Attend staff meetings, Quality Assurance Meetings and educational activities.
 

-  Compliance with RCP policy and procedures in accordance with state and federal regulations relating to patient care and services. (Medicare, OSHA, or other governing agencies).
 

-  Compliance hours may be incurred in order to meet the needs of the program.
 

-  Understand and follow RCP patient and employment policy and procedure. 


Qualifications:



Graduate of an accredited school of nursing.


High school graduate or equivalent/medical background.


Current licensure as an R.N. by the state in which the dialysis facility is located.


Demonstrated ability to function in a leadership position and to perform in new and emergent situations with sound judgement.


At least one (1) year RN experience in dialysis.


6 months peritoneal dialysis experience


Management experience required


Training/teaching experience in the education of patient, family or staff


Demonstrated working knowledge of the English language and ability to communicate verbally and in writing.


Ability to perform all range of motion; walking, sitting, bending, reaching, stooping as required for performing dialysis therapy. Physically able to lift patients from stretcher or wheelchair or assist patients from stretcher or wheelchair as necessary. Able to lift objects weighing up to 35 lbs.


Current CPR Certification.
